Why does my dog scratch his ears so much?

My 2-year-old Labrador keeps scratching his ears constantly for the past week. I noticed some dark discharge and a bad smell coming from his ears. He shakes his head frequently and seems uncomfortable. Could this be an ear infection and does pet insurance cover ear infection treatment?
